Accident analysis: don't fall when you are very runout; many bad things can happen. Yet you probably climb faster if you place less gear. They knew the risks.

Still an open question what minimal gear to bring or setup to use which would have helped enable Bill to descend the rope for the full self-rescue. It sounds like 2 more prusiks would have been enough to do this.
