Not a whole lot of information, but most likely easily preventable.

It’s not clear he was rappelling, there is some talk that he was scrambling at the top of a wall. He was known for developing new routes in our area. A very sad day in Central Oregon, he worked at our local gym. I’m sorry to his family and many friends.

From reading the report,it is most likely an anchor failure or attachment disconnect if he still had the rope and climbing gear with him

From the update:

Reed "was not actively engaged in climbing" at the time "but was equipped with climbing rope, harness and other gear," Johnstone said, adding that a "preliminary investigation shows this to be an accidental fall of approximately 200 feet.”

“He was still wearing his rope," Johnstone said. "He had his climbing gear still with him. It did not appear that he had actually started to make any climbs at this time. He was maybe scouting for an area.”

There I no evidence he was rappelling or doing anything but 4th classing.

My sincerest condolences to the family and friends of the young man. It hits particularly hard when it is a promising young life. Sounds like he was a good man.
